Liability Waiver for Volunteers

If you're volunteering, HSH needs to have the "Release and Waiver of Liability" signed by you and on file with us.

1.  Please download the waiver shown below.  

   • Click the small arrow inside the square in top right corner of the waiver shown below.  It will open a new window.  Once you see the waiver in a new window, click the download button to download a copy of the waiver wto your computer.

2. Print the waiver and sign it, filling out all blanks.

3. Send the waiver back to HSH.  

   • You can scan & email it to HSH using "Events@hshobart.org"

   • You can take a photo of both sides and email it to HSH using "Events@hshobart.org"

   • You can drop off your signed paper copy at the shelter. 

   • Request a digital copy to sign by emailing "Events@hshobart.org"
